In the state of Utah I hate to say maternity as of right now is not covered very well in the individual market. Most policies have a $7500 separate deductible for maternity or are not covered at all. If you are thinking about having a baby I would highly recommend looking at an HSA plan. The reason for this is a normal pregnancy is not going to cost $7500. If you make arrangements with a hospital when you find out your are pregnant and if you are willing to use a mid-wife you can negotiate the cost to under $4000. If you use money from your HSA that is money that is tax deductible so it brings down the effective out of pocket cost. Once the baby is born they are automatically covered under your policy so if the baby has any problems the cost will go towards the deductible. Since the passage of the Affordable Care Act baby wellness visits are also now covered no deductible no co-payment

Question #2: How much total could I be responsible for each year if the worst case scenario happened

It is important to ask if the deductible you choose is times 2 or 3 people in the family and if the deductible counts towards your out of pocket limit for the year or is it separate from the deductible amount.
This can make thousands of dollars difference to you in the event that more than one person needs the insurance in any one year. It does also effect you if you are the only one on the insurance just not as significantly.
